MEDICAL INSURANCE
All Full-Time CEXEC Employees have a choice of obtaining medical services from a PPO Preferred Provider Organization (In-Network) or going outside of the PPO Network to a Non-PPO (Out-of-Network) provider.  CEXEC maintains a Point of Service Plan, which means you may elect either option to suit your needs. CEXEC’s Employee Health Plan includes the PPO option through either Alliance PPO or Beech Street PPO, depending on the employee’s location.  Deductibles apply for the use of Out-of Network providers. A prescription plan comes standard with our Medical Plan and is currently being offered through PharmaCare.  Coverage levels are 100% in network, and 70% out of network, and various employee premium rates apply.

DENTAL CARE
The CEXEC Employee Dental Plan is a basic indemnity plan that covers a percentage of applicable dental work. A six-month waiting period is applied to basic dental services (Level II) and major dental services (Level III) for new hires. Benefits are a 100% of preventive services, 80% of basic services after the deductible, and 50% of major services after a deductible.  Various employee premium rates apply.

LIFE AND AD&D INSURANCE
Coverage is provided at no cost to CEXEC employees and is equal to one times current annual salary up to a maximum of $150,000 and a minimum of $50,000.  Supplemental insurance is also available at reasonable group rates in increments of $10,000 to a maximum of $500,000 for employees, $100,000 for the spouse and up to $10,000 for dependent children.

FLEXIBLE BENEFIT
The CEXEC Section 125 plan enables pre-tax employee-paid contributions to cover medical and dental insurance premiums as well as for Plan contributions to Medical Reimbursements and/or Dependent Care accounts.

DISABILITY PROGRAM
Short Term -no cost to employee, eligibility in 30 days.  Fully funded by CEXEC.  Benefit is 60% of salary up to a limit of $800 per week as of the 8th calendar day of absence for illness or as of the day of injury for accidents for a maximum of twelve weeks.  Long Term -no cost to employee, 100% of premium paid by CEXEC.  After a 90-day elimination period, benefit paid by Principal is 60% of salary up to a maximum of $7,000 per month.  Reduction of benefits at age 65.

EMPLOYEE REFERRAL
Cash bonus awards for referring qualified candidates to CEXEC.

EMPLOYEE EDUCATION ASSISTANCE PLAN
In addition to $3,000 of college level education assistance per year towards accredited expenses, employees may also be eligible for job-related training assistance that is subject to management approval and certain repayment schedules apply. 

401(K) & RETIREMENT
All qualified employees are automatically enrolled into the 401(k) Plan after the begining of the calendar month after 6 months of service and attainment of age 20.5. CEXEC will match the participant contributions equal to 50% of the first 3% of the annual W-2 salary. Participants shall be 100% vested in pay deferrals made from their salary and, after 3 years of service, will be 100% vested in employer funds contributed in their behalf.

ANNUAL LEAVE
Annual leave encompasses all types of accrued leave. You may use this leave for sick leave or personal/vacation time.
