Medical Insurance
CEXEC and its employees share the cost of medical insurance premiums. CEXEC bears
approximately 70% of the premium cost of employee coverage and 70% of the premium cost for
dependent coverage. Employees have a choice of obtaining medical services from a PPO Preferred
Provider Organization (In-Network) or Non-PPO (Out-of-Network) provider. This is a Point
of Service Plan which means employees can elect either option at any time. A PPO is a
specific network of hospitals and physicians who render health care services to
participants. A prescription drug card is provided as well as a mail-in
pharmacy option for long-term drug prescriptions at a reduced price. CEXEC accepts creditable medical coverage
certificates from former employers to reduce the preexisting condition exclusion period as
per the Health Insurance Portability and Accountability Act (HIPAA).

Dental Care
CEXEC bears approximately a 70% share of the cost of employee and dependent coverage. A six-month waiting period is applied to basic dental services (Level II) and major dental services (Level III) for new hires. The calendar year maximum for dental services is $1,000 per person per plan year or $500 if employment date is July 1 or later.

Life and AD&D Insurance
Coverage is provided at no cost to the employee and is equal to one times the employee’s current annual salary up to a maximum of $150,000 and a minimum of $50,000. Reduction in benefit at age 70. Supplemental insurance is paid for by the employee and is available for the employee, spouse and children at very reasonable rates.

Flexible Benefit Plan
This plan enables employee-paid contributions for unreimbursed medical and dental to be in pre-tax dollars (premium conversion). Employees are also able to defer
pre-tax dollars for contributions to Medical Reimbursement and/or Dependent Care accounts.
Supplemental life insurance can not be paid for with pre-tax dollars.

Disability Program
Short Term -- no cost to employee, eligibility in 30 days. Fully funded by CEXEC. Benefit is 60% of salary up to a limit of $800 per week to a maximum of 90 days. Long Term -- no cost to employee, eligibility after 90 days. Benefit is 60% of salary up to a maximum of $7,000 per month. Reduction of benefits at age 65.

401(k) & Retirement
Employees are eligible to participate after six months of service and attainment of age 20 ½ .
Enrollment dates are January 1st, April 1st, July 1st, and October 1st. CEXEC will match
the participant contributions equal to 50% of the first 3% of the annual W-2 salary.
Participants can contribute up to 60% of their salary. Participants shall be 100% vested in
pay deferrals made from their salary and, after three years of service, will be 100% vested in
employer matching funds contributed on their behalf.

Annual Leave
Annual leave encompasses sick leave and vacation at the following accrual

Job-Related Training
Assistance is provided for training directly related to the employees current job
and is subject to supervisor approval. Full or partial payment is provided. Should an
employee leave employment within 12 months of completion of training
course(s) for which
the Company has paid fees the company will be reimbursed according to the following
schedule: 1-6 months = 100% repayment; 6-12 months = 50% repayment.
